Assessment Summary Report (ASR)

The Assessment Summary Report (ASR), formerly known as the Corrective Action Plan Report (CAP or CAPR), is a concise summary of the assessment results which is designed to clearly communicate a facility’s overall performance to management and specify the steps management must take to ensure full compliance with TCCC’s Supplier Guiding Principles. The document serves two primary functions; (1) it provides a user friendly set of instructions for a facility manager to achieve compliance and (2) it establishes a contract that defines the conditions of the relationship between TCCC and the supplier.
The ASR contains the following sections:
1. General assessment information- This section identifies the TCCC business unit responsible for initiating the assessment, facility name and contact information, members of the assessment team, the number of total employees, the number of
employees present on the day of the assessment, and the number of employees interviewed.
2. Violation score and rating- This section provides the facility’s numerical score and its corresponding rating of Green, Yellow, Orange, or Red (see “Color Rating System” on page 26).
3. Findings - This section provides an itemized list of each finding that is a violation of local law, the applicable law, appropriate corrective action to be taken by facility management, and an agreed- upon time frame for remediation. Each finding is assigned a code number from the violation matrix, which indicates how the finding affects the facility’s violation score.
4. Suggested Best Practices - This section lists findings that are areas for improvement but are not in violation of local law. Findings in this section do not affect the facility’s assessment score. While corrective action of items in this
section is not required, it demonstrates the supplier’s commitment to a favorable working environment for its employees and increases the probability that TCCC will maintain its relationship with the supplier for its future needs.
Using the ASR Template
When using this Assessment Summary Report template in conjunction with the Violation Matrix, please take note of the following design features and user instructions:
In the product category box, please select a product category from the drop- down menu that best represents the product or service produced by the assessed facility. Be sure to review all possible choices before selecting a product category, as one category may appear to be the correct choice, but there may be a more accurate choice. See the following important notes below:
1. If a facility is producing bottles of orange juice, you would categorize this as
"Beverages - Alternate Beverages" rather than "Fruits - Orange Fruit or Orange
Juice". The fruit category is reserved for suppliers of actual fruit or fruit juice and
NOT bottlers who fill bottles with the fruit product.
2. It is important to distinguish between a bottler (a facility that fills bottles) and a
supplier that produces bottle containers. Facilities that make glass or plastic
bottles but do not fill them with beverage product are to be classified as
"Packaging".
3. If you classify a supplier under the "Services" category, then the supplier must
NOT be producing a tangible product, but rather a service such as lawn care,
technical support, accounting, or other service function.
